Our Services:

Home PharmAssist’s services are designed to simplify your medication routine.  Our pharmacists provide the following in-home services:

 

1. Complete medication work up (60-90 min):

- Medication reconciliation (checking for interactions or duplicates) and patient education on each medication.  

- Personal Health Record binder including a health summery sheet and space to keep other important health documents.  

- Personalized pillbox selection and 1st 2 week pill box set up

- Proper disposal of expired or unused medications

 

2. Home PharmAssist (HPA) membership - Bi-weekly visit (30 min):

- Pill box set up

- Medication adherence monitoring

- Medication review/update medication summary sheet (accommodate medication changes).  

*Option for medication refill ordering and prescription pick up (additional fee).

 

Patients will be able to "consent" to having after visit summaries provided to an outside party/family member.

 

 

HEAD PHARMACIST/OWNER:

Dr. Jennifer Smith has 25 years of pharmacist experience, having received her Doctor of Pharmacy degree from the University of Iowa in 2000.  Growing up the daughter of independent pharmacy owners, Jennifer learned from a young age that treating patients with care and a personal touch goes a long way in assisting with their healthcare needs.   Jennifer and her husband Peter are active in the Northshore community and are currently raising their 4 children in Wilmette.
